To obtain Vital Records

Town of Massena Clerk's Office
hours Monday - Friday, 8am - 4:30pm

Birth Certificate requests For individuals born in either the Town or Village of Massena.

Death certificates of individuals who have died in the Village or Town of Massena.

Marriage Cerificates of individuals who have purchased their marriage license in this office only.

Requests can be made in person, in writing, or via the internet by credit card.   We have the following forms for you to download...

In person: Town Clerk's Office, 60 Main St. Room 3, Massena, NY 13662.   Need identification, download request form/fill out, $10.00 fee payable by cash or check.

In writing: Town Clerk's Office, 60 Main St. Room 3, Massena, NY 13662.  Need photocopy of identification, download request form/fill out and notarized, $10.00 fee payable to Town Clerk.

Credit cards only, credit cards must be in record holder's name or be a parent of the individual on a birth record, go to www.vitalchek.com, need photocopy of identification.  (Please enlarge photocopy of license so the info is legible on fax)
